Install jqGoogleForms

Create a Google Form by clicking "CREATE" -> "Form" at http://drive.google.com, add all of the fields that you desire. Note the form key, found in the URL between "/d/" and "/edit" of the form creation page:. docs.google.com/forms/d/ (YOUR FORM KEY) /edit.
Open Chrome (or another browser that can view source)
View your form live
On each form input or text area there will be a "name" attribute, set to something like "entry.1234567890"
Gather all of these names, and keep in mind which field they are associated with
Use the "sendFormData" method, passing in an object containing all of the data you wish to send in the format of {name:data,name:data}
